Sub Glottal pressure...really. It's your upper body strength-- air builds up. Some people can't control the air output--and "voice" the pressure. I work with Laryengectome patients that lose all of that..it totally effects your upper body strength. Now, some muscle heads do it to DO IT..but a lot of them can't control it. Seriously.

In Kendo, the 'grunting/screaming' is called 'kiai'.
My understanding (probably lacking) is that it is an expression of spirit and is used as a means of facilitating the gathering and expelling of energy. I think Japanese martial artists have become consciously aware of what guys in the gym discover naturally - that 'grunting' helps exert a greater effort and allows more vigorous and effective training.
People who hold this in seem self-conscious and too concerned with the opinions of others. Of course it sounds stupid "Urgh argh haaargh" - we don't make noises like that in the dojo or gym to brag (how could such terrible noise even be considered bragging?) we do it because it serves an effective function. And it serves it well. If, in the gym, you aren't almost by necessity grunting and groaning, then you aren't putting in a worthwhile effort. Put the protein shake away and go join and aerobics class with the other girls.

Because they're lifting extreme weight, BRO!! It's manly to grunt and groan. What about tennis players? Back and forth back and forth UH! AHHH! UHH! ARHH! Actually this is more prominent in female tennis players, from what I've observed. They're not seeking attention obviously. Groaning just gets out some of the exertion. Have you ever lifted a heavy box and groaned HURGGHH! It happens. Some people let it out vocally.. some turn red.
